To make it easier to do business with us, Prairie Materials offers a variety of payment methods, including COD, Visa, Master Card, American Express, and Net Terms (on approved credit). To apply for credit with our company, please download and complete our Credit Application PDF. When you have completed the application, print it and mail it to our credit department:

To expedite processing, please contact each of your listed credit references prior to mailing this application to Prairie.

Waiver of Lien

If you are working with us on a current project, you may need to prove that you have paid for your materials before you can get paid. The most efficient way to verify compliance is to submit a Waiver of Lien Request through our website. Once we receive your request, we verify your status and mail a waiver of lien letter back to you as soon as possible.

Sales Tax Exempt Forms

If you are working on a project that you believe is exempt from state sales tax, we require a certificate verifying that status. Please select the appropriate sales tax exemption form below and submit it to us before the project starts.

Clean Construction and Demolition Debris (CCDD) Acceptance Forms

Prairie Materials accepts uncontaminated soil and uncontaminated soil mixed with clean construction or demolition debris (CCDD) that complies with EPA regulations at a number of special debris recycling locations .

In Illinois, any material we accept must be certified to be uncontaminated using the appropriate Illinois EPA form.

All jobs must be pre-approved – NO EXCEPTIONS!

Please complete the CCDD Application Form  and appropriate Illinois EPA form and email it to ccdd@vcimentos.com for approval.

Make sure you include all supporting documentation.

Agricultural and Residential Properties or Projects

Uncontaminated soil from a site that has not been used for commercial or industrial purposes, such as residential and agricultural soil. These projects may be certified by either the site owner, operator, or a licensed professional engineer.

Complete the Illinois EPA Form LPC-662 and email it to ccdd@vcimentos.com for approval.

Commercial or Industrial Properties or Projects

When soil is removed from a site that has been used for commercial or industrial purposes, certifications must be made by a licensed professional engineer. These projects include most roadway, utility, and right of way projects.

Have the engineer complete the Illinois EPA Form LPC-663. Email it to ccdd@vcimentos.com.
